It's the same Ultima Weapon, it's been at some point thrown into the Rift just like Gilgamesh - this incarnation predates its FF VII appearance.

Cloud led a long life of heroism, but then a strange, radioactive meteorite crashes next to him and pollutes him. He is mutated into a creature of evil, and attacks all that attack him.

Or, they just couldn't be bothered to make yet another sword.

It's not Cloud's Ultima Weapon, it's Ultima Weapon's Ultima Weapon! Ultima Weapon belonged to Ultima Weapon in the first place. Then Cloud came along and stole Ultima Weapon from Ultima Weapon by defeating Ultima Weapon.

It's not Cloud's Ultima Weapon, it's Ultima Weapon's Ultima Weapon! Ultima Weapon belonged to Ultima Weapon in the first place. Then Cloud came along and stole Ultima Weapon from Ultima Weapon by defeating Ultima Weapon.
