Mine Mako from the Lifestream. From the Lifestream, Sephiroth began expirimenting with his clones, and that's why most of them would shake sit on the floor and stuff and end up moving towards Sephiroth. One of his clones he was able to control every movement, and that was the "man in the black cape"

That's one possibilities, but there's two other theories. One of them, that the "man in the black cape" was Jenova-in-disguise, is also more probable.

Actually it depends whether you count Ancient Forest because although it is possible to get there through a different method you can raise a green/black/gold chocobo and get there before disc 3. It all depends whether you mean that can ONLY be reached by chocobo or ones where using a chocobo is a handy trick. Although before disc 3 it is ONLY attainable by using a chocobo so you could argue that it works either way.

EDIT: Oh and I distinctly remember Sephiroth saying his surname was Williams
