Well, a few art-house theatres might show it a couple of times... but outside Japan, nearly everyone will have to watch it on DVD.

i'd say it would be rated 15 over here in England. But since it's gonna be on DVD, parents are gonna buy it for their kids anyways...so, it shouldn't really affect the sales...
