I think the gray wolf is a representation of Cloud's lone-wolf nature. At the end of the film it disappears and cloud confesses "I'm not alone."

A lot of people seem to think it could be the spirit of Zack, however.

The wolf has been confirmed by Tetsuya Nomura himself—in the audio commentary of the Advent Children DVD—to be a symbol of Cloud's guilt.

Notice it appears in places where he's failed people. Aerith's grave, Zack's grave, over Tifa's prone and beaten figure, etc.
