Was reading about Flanderization on TV. Here's on about Star Trek:
Warp core breach? When first mentioned, it was so absurdly unlikely due to all the redundant safeguards that the characters couldn't understand how it even happened; but before long, the writers were using "warp core breach" as a go-to danger so often that it seemed a core would breach if you even looked at it funny.
