If you've watched the subtitled anime, Lust generally calls Ed "boy". Winry's grandmother generally calls him "chibi" or "chibi-chan". The suffix -kun in a name implies friendship, and is also often used with boys and men. You often hear it at the end of Ed's name when he's being referred to by one of the State Alchemists. Consequently, Nii-san is a formal address used by Al when refering to Ed.
