I'd imagine that there are people who would buy it seriously and people who would buy it as a joke.

Depends on the context of the site that is selling the t-shirt. It could be considered a satirical joke, but it would be a very bad one.

It's a joke if people find it funny. Whether or not it's intended that way. I for one find the concept of a 'Proud Republican' to be hilarious, so I guess that makes the answer to your question a Yes.
