I got all the items required according to gamefaqs... i sold 3 mythril, 3 corpse flys, and 1 aquariuas gem and it didn't come up, am i missing something or whats the deal?

4 aquarius gem are needed

You should reset, otherwise the mythril might be used for something else entirely
