The hours I've clocked on Guild Wars knowing for a fact that I've only been afk with it on once (it was about 10 hours).

Probably Omega Weapon from FFVIII without the use of the Hero drink. Also, finishing The Revenge of Shinobi was an acomplishment in itself as I was a young age doing so.
I've finished Fable about 15 or 16 times, and 2 times with friends doing parts, and Fable TLC about 3 times. And I finished Fable TLC 8 times in 4 days (playing from about 10AM til 10PM)
I accidentally activated a cheat code by just bashing the buttons absent-mindedly. I never found the cheat in any books or the internet after.

Finishing my first game ever, Bubbsy for the SNES. Made was that a great day and a great game.
I beat Dragon: The Bruce Lee Story on the Genesis. I felt pretty freaking ace after that.

I guess beating Dirge of Cerberus in one sitting. Kingdom Hearts II in like a day, or close to it, with getting the ultimate weapons.
I dunno, I don't really see anything to do with video games as an accomplishment. Unless ofcourse you work for a game company that just won an award for a game you helped design/produce/whatever.
Beating all the sidequests and secret bosses in GS2, including Dullahan. And I also completely did that island (Forgot name) on gs1 and beat deadbeard.
Probably either crafting a perfect one-handed sword in Vagrant Story, or getting as far as I did on my Final Fantasy X NSGIENC game, since that was an original challenge which no one had ever attempted, prior.
