Right click one of the columns that are still there. You should get a list where you can check the columns you want and uncheck the ones you don't want. If "Type" and "Date modified" aren't in that list, click "More..." and you should find them there.
It might be a bit different depending on Windows version, but should be pretty consistent if I remember correctly.
