It's Cloud from the future, after all the Midgar pollution gets to him.
He's riding a mutated Nanaki.
Poor Nanaki :(
Sephiroth returned, killing Cloud and taking his sword, maybe his masamune got broken and needed Cloud's instead
It's the same Ultima Weapon, it's been at some point thrown into the Rift just like Gilgamesh - this incarnation predates its FF VII appearance.
