Sometimes a Game Over screen isn't enough. Sometimes a game has you sit and watch through just how badly you smurfed up and made everything go horribly wrong, in the most depressing ways imaginable.
I actually really like Bad Ends in a game when done right. They can add so much to a story and when the story is emotionally involving... holy crap a bad end can hit really damn hard.
A prime example would be this:
So what particularly notable Bad Ends have you encountered in a game?
The Suikoden series has some. Notably Suikoden II if you decide to leave basically and Suikoden V if you take some bad offers
Are we talking about Bad Endings or more involved Game Over screens?
In terms of Bad Endings, Persona 4, BoFII, and the Suikoden series know how to punish you for making bad choices to get their bad endings.
In terms of you died and now here's your Game Over screen, I don't think anyone really beats MegaTen on this one...
Of course the best one which serves as both a special Game Over for dying and serves as an actual bad ending goes to Chrono Trigger.

Persona 4 Golden's Accomplice ending is probably my favorite in this category. Creepy. I wish we could get a brief chapter on what happens after that.

Final Fantasy X. And also Fallout 3. God, what a disappointment that was.
Tactics Ogre is pretty much Bad End: The Game.
I found most of Fate/Stay Night's Bad Ends to be rather plain. But there's one that stood out to me like no other...

Right at the end you fight Saber, and once you beat her you're given a choice to spare or kill her. I had the correct choice spoiled for me, but I still wanted to see what happens if you choose to save her.
Wow. Just wow. I don't think there's a darker ending in this game. Saber strikes down Tohsaka before she can get close enough to Sakura and Sakura proceeds to submit Tohsaka, and then Shirou, to endless torture. Well congrats, you smurfed up horribly.
One of my favorite execution of bad ends also comes from Tomoyo After, where every Bad End has a simple "happily ever after" and the True/Good End is just depressing as hell.
Nothing beats outwitting a supernatural entity and cheating death itself, only to get run over by a couple of drunks at the end.

Final Fight has what is probably the second best game over in all of gaming.
For bad ends? I have a tendency to play to get a good ending, so there are very few games in which I received a bad ending. I got bad endings in the Ace Attorney games for trouts and giggles, but the best one I've personally seen was the one for Persona 4.
Well in terms of just bad endings. This is easily my favorite of the endings from Silent Hill 2.
