What are ALL the endings I can get and HOW do i get them?

You can just see what happens from here. I did know a good site, but I can't remember it.

This is all off the top of my head, so feel free to correct it--
There's the bad ending. Just lose the final battle.
The normal ending, don't do anything special at all, but win the last fight.
The good ending, you have to push X at a few choice places and win the last battle.
The perfect ending, get 100% completion, push X at the choice places, and win the last battle.
