Percentage Question

I played through the first time, joining up with the Youth League, didn't solve the Mi'hein Road Mystery, fight and defeat Trema, and did not find or complete any of the secret dungeons through the Chocobo Ranch and got 94%.

I played through the second time, joined up with the League again, solved the Mystery, got up to Paragon (but did not defeat him), and still didn't complete the secret dungeons. This time 98%.

I just started the third time, joined up with New Yevon and got 99%. I then fought my way through Mushroom Rock Road and got 100%.

I thought you could only get 100% by defeating Trema and finding and completing the Chocobo Ranch and secret dungeons. Is my game just glitchy? I am going to try and rush through and see if I get the perfect ending.

some things are optional for 100%. its a bit strange but nothings wrong with your game.
