LOL Just as I'm about to go work, ok so I need my cell phone, wallet, keys, mp3 player and normally I'll have sunglasses in a jacket pocket during the summer and most days I'll have a bag with a book in it for tube journeys and speaking of which I also carry my oyster travel card.
Phone, wallet and iPod. 90% of the time I'll take a pen with me, just in case.
When I go by train: Grey shoulderbag, iPod with Sony MDR-V700 headphones, wallet, cellphone, sunglasses, glasses in etui, chapstick, the book I'm reading or my NDS, toothbrush etc. and keys.
If by bike: Same shoulderbag, bikechain, -pump, -lights and -tools (you never know, sunglasses, chapstick, wallet, cellphone and keys.

Even if I don't carry my bag, I carry, at least, my Hello Kitty wallet.
Sometimes I don't leave the house without Tiki and Tobi.
Things I check for when leaving for work: Keys, bus card, belt, work passcard, wallet, phone, belt, mints. For just leaving for wherever, I need my keys, and preferrably would have all those other things, but the keys (and bus card when travelling) are obviously the main part.
