any mod or admin can change your avatar, but I can't imagine why anyone did, unless it was offensive or something.

also, it might be a problem with the database storing your custom avatar or something. I know sometimes the settings can get screwy. Blame cl_out.

It wasn't me, but I do know the cause :p

it can only be attributed to...human error.

Although, the specific "human" involved is widely reputed to be a robot.
