I know that 'What in the Sam Hill?' is a figure of speech, and that someone might have said it, but I don't know of it as the name of a town.
It was the figure of speech, right? If that's the case, then it's a peculiarity of the English language, and probably wasn't present in the Japanese version--meaning that it's not a town name but an anachronism, like Squall saying 'God'.

Actually, figures of speech are not universal. They don't all translate perfectly. I challenge you to find direct translations of most idioms, for instance, that make sense.
The game was originally in Japanese, and was translated into English. It wasn't just a direct word-for-word translation, but a translation meant to give the meaning behind each line, as it would be in English.
So, sorry, MJN, but I am stating otherwise.
That is not translating it is dubbing.

Actually it would go like this:

It is a composition of the words win +hill so maybe the word Sam being a name appears as an English word on a Japanese script.
There is no real reason to believe the common words weren´t translated literally just because they appear on a speech figure.
For instance Winhill could appear as Japanese with the words win+hill glued and Deling as an English name.
