I doubt it has anything to do with the forth place you visit. What at fort Condor looks like anything else too? Also some people see the forth place as Junon cause you don't really have to go to Fort Condor until later in the game. And I still have nothing to guess...

Edit: hmm, maybe it's something with Cloud's & Sephiroth's katanas, Yoshiyuki, Murasame and Masamune? Maybe the "4th" is the last battle in the game, and it's the Masamune? It looks like two others, only that because I've changed beats me.
I will leave it for a few more days then I will give 1 hint. That hint will be big enough to narrow it down. I will say that the clue is very fair, and isn't something horrible like "the 4th town you visit" or "4th weapon you buy"

When tifa and cloud fall into the life stream, Tifa helps cloud gain his true self. Where there are 3 different stories which they follow. 1 of them is truth and after truth is met they all merge with the 4th bringing back Cloud's true self.
and your death can be arranged.
Now that Cloud has his true self and he cannot be controlled, he can finally fight Sephiroth without fear of being used against his comrades?

Could it be Yoshiyuki? It looks like Murasame and Masamune and it's at the place where you get the 4th great materia ...

The clue must be answered in all areas. There are lots of "4ths" about and "deaths" but there is only a few things that can apply to "I look like 2 others, that is because I've changed" [all parts of this are literal and not vague]
Bahumut Zero does not literally look like Neo or Bahumut, even though it is a bahumut summon. Although this was an excellent guess.

Going by Tifa's logic suggests:
1) It's a monster that occurs in several palette-swapped versions, three most likely.
2) 4 might refer to the endgame, so it may be in the Northern Crater.
That combined with Death, suggests to me Death Dealers, which can cast the instant-death Roulette, and drop Kiss of Death, and contain the word in the names.
