I always liked the frankness of Resident Evil's "You Are Dead".
I’ve never played Friday the 13th, but I watched the AVGN’s review of it and remember the game over screen being particularly stupid... something like

Oh man, I forgot about CT. Showing the consequences of your failure is a great idea. Only problem is it doesn't work on games where you will die a lot.
*Imagines a Mega Man game with a 5 minute cutsceen every death*
It happens in Strange Journey too. It's not as long though, but you see the Schwarzwelt swallow the Earth when a demon kills you...or turns you to stone...or charms your demon and it kills you...ect.
I liked the game over screens in Tenchu 1, 2 and 3. Every character had it's own (sometimes not that pleasant) visual and poëm. It made death look useful and such.

Always thought it was amusing when you die in a MGS game and the person you have on the codec radio is all like "Snake? What happened!? SNAAAAAAAAAAAAKE!!!!"
Gotta love it.
Disgaea has pretty good game overs.
since each boss fight, if you die then the story would unfold differently and they'd give you a little epilogue of what would happen, but then they're nice and let you restart the game with all your stats =D
yeah, MGS has the some of the most enjoyable Game Over scenes. I'd kill myself just to hear them sometimes =]

Yeah Metal Gear Solid really takes this. In addition to the great presentation of it all, you have "Fission Mailed" in MGS2 and Time Paradox in MGS3.
The way it was done in MGS4 is enough to give you a heart attack - literally seeing someone's life flashing before their eyes, with some horrifying stuff like "Snake!? YOU CAN'T DIE!!!! Snake!! SNAAAAKE!!!"

I would have to say haunting ground has the most wtf game over i shall give a link and you will see they are creepy/ eeww sounds Wait for danella she is the by far the creepyest :D
