I've read the entire Redwall Series (18 books) at least 6 times through in my lifetime so...I guess that would be it. Getting a bit old for it now but it's been one of my favourite series since I was 11 so I still read it for nostalgia purposes.
Twice for Rainbow Six, though once I actually finish the Dune series (basically once Sandworms of Dune is released and I read it), I think I'll go through and start reading them all again. That's a series that is too good to read once, and that you can't get everything out of the first time through.
I rarely, if ever, read a book more than once. I have so many books collecting dust in my to-read pile that I can't afford to waste the time. However, I did read Michael Crichton's Sphere three times. That was a great book.
