In Xenosaga 2 after defeating one of those Erde Kaiser bosses I didn't save and ran into another creepy optional boss and died. The Erde Kaiser wasn't a difficult fight but it was a pain in the a.

FFXII I lost count of the number of times my game stalled after a bout of levelling.

FFX getting killed by a Marlboro in the Omega Ruins after gaining 80+ Sphere levels and several items required for status effect nulling slots on weapons.

Pandemic 2, and the countless times I've managed to stealthily infect every country with my disease that has no symptoms to begin with... and then freakin' Madagascar closes its borders and shipyards.

This has happened to me maybe twenty times now.

I was playing Shadow Hearts and I was having trouble with a boss fight, it took a few tries, but finally I defeated it and prepared to see a very nice in game movie where (SPOILER)Seraphic Radiance destroys Shangai and that's when our cat walks in, steps on the controller that was on the floor and skipped the scene.
    Argh.

And recently, while playing Suikoden III, somehow, I neglected to save during the entire 1st chapter of Chris or the game screwed up, I don't know how that could have been possible, but, yes!, having passed it and done my first tactical battle a few times, because I just did not know what the heck was I doing and had to go through a bunch of scenes every time I lost that damn tactical battle, I found that I had to do the entire chapter all over again and of course, the tactical battle again.

Hmm gah moments happen loads to me, like 3 days solid FFVIII without saving apart from the disc swaps and then my playstation crashing due to heat....great!

Halo 3 Legendary Campaign Single Player, the damn arbiter once kicked a crate in to my head killing me on the flood levels in new mombasa. Thankfully it was just after a check point.

MGS2 Sons of Liberty European Extreme difficulty mode, this bears no explaining it's the most infuriating mode available you only need to be seen for a game over.

Mass Effect, I tend to do missions and forget to save so I rush through them and I'll do something ever so slightly stupid like touch lava with one wheel of the mako and die having no choice but to revert to the last auto save file, generally this is planet fall on the mission.

I've also had gah moments in Ace Combat (spectacular dog fight to find out I am at 20 feet after pulling out of my dive and to either somehow have someone on my 6 or to get clipped by an AA gun distracting me long enough to slam in to a surface) And many many many more.

Digimon World: Dusk, I just beat the elite Mega Digimon that protect the world from the shadows which in turn unlocked a few new Digimon as random encounters in some old areas. Did some leveling on my Digi-farm and a few Digivolutions and went to check out the new things... I got a lv. 93 Armageddimon who wiped the floor with my best party configuration... so much time wasted. I need to learn to save more often...

Umm, you don't get a game over when you lose in that game, you wake up in your base. The only penalty is you lose some friendlyness from your team, but that's easily reobtainable.

    I just remembered one. :P

When I was like seven or eight years old, I was playing Duck Tales (the awesome old NES game) and I was doing really well for the first time ever. xP I had almost all of the treasures and I was in Transylvania and I got stuck in a wall. >_> I was very annoyed at that.

Hm, when I came across a saving glitch on LoZ: Twilight Princess I was really pissed. I'd saved and turned it off, came to carry on playing and it restarted me on the other side of the bridge (which had been removed after I'd crossed it) so I had no way to get across and continue. I'd already done one temple and loads of things in Twilight so I was really pissed that it was all for nothing.
