She was in a boat far out on the ocean, with her child on board, she jumped out for a swim, and forgot to put out the ladder before she jumped out. The walls on the boat were too tall, so she couldn't get back in without the ladder available, although she tried scratching her way up. This is part of the plot of Adrift 2 and also a true story, apparently.
