Sounds to me like Jenova DEATH. Its palette swapped with LIFE and BIRTH, and is also 'changed' into Sephiroth's form, making this the literal reason they look alike.
I can't think of a sensible reason for '4th' though, except perhaps the fourth continent, but LIFE is there too so thats not much help. I'm really only guessing DEATH over the other two because the word is in the clue.
If I'm right someone else can post... though I feel bad if it is as this really isn't solved.

Is it Hojo, when you fight him and he turns into three possible forms? The 4th might be Sector 4, I'm not sure where that Cannon was setup.
nope :P and the cannon was set up in same area as you start game before you jump the bridge. Sector 8 I believe...
at least that is the route you take to get to Hojo.

and your death can be arranged. - and he will counter with Chocobuckle

again the clue isn't vague...it is quite literal
Find me at the 4th, being LV4 suicide would be a bit mean on people.
Is it Staniv, the fourth boss Yuffie fights in the Pagoda. His model has been used on other monsters. Not too sure about the death can be arranged bit for him though, as he is pretty weak - sounds like it suits Godo more, but he is on the fifth floor.
So I'm going to say Staniv :P

You meet him on the 4th level of the pagoda. You challange him, he can arrange your death, or at least he tries to. Staniv CHANGES into a monster which has 2 model textures.
In other words "I look like 2 others, that is because I've changed" is literally that he changed from human to the monster

is it Sephiroth? The real him is in the bottom of the Norther Cave, he has to wait till all the "clones" get there. Through the game all the clones are searching for him, and when Cloud gets there Cloud... again gives the Black Materia to Sephiroth which unlocks him once again from his "slumber", if you will, and the weapons are released.

Nope. Not any of the Weapons or Sephiroth.
But they fit the riddle perfectly, I hope I dind't make the riddle too vague, this might be a tricky one, but I tried not to make it too easy of course.
I will no doubt be supplying hints soon me thinks
