Nominally, I live by myself, so of course I leave the door open
If I'm in a hurry I'll leave the door open. This is usually late at night after I've had about a gallon of soda so I'm usually safe from surprise visitors.
I rarely leave the door open when I go pee... but that's because I have a hard time peeing considering my failed kidneys, so I rarely ever go. I always have the doors closed and locked when I use the toilet though!
Apparently, I have been declared banished.
Yesterday BJ tried to block me from going to the bathroom when I really had to pee so I punished him by peeing with the door open. I do it all the time! It's like a little adventure.

I just swing it shut on the way in, if it doesn't close then it doesn't matter. Always have my back to the door anyway.
Door closed. You never know when someone with a camera will jump at you.

I close the door, even when alone, out of habit. I enjoy my privacy.
If I'm home alone (which I mostly am) I won't ever shut the bathroom door unless I'm having a shower, and even then it's only to trap the steam so it's not a cold shock to leave the shower. If I'm with my closest family I'll close the door but only sometimes bother locking it. If there are others nearby I'll usually lock it.
If I'm in the flat by myself then anything can and will be done door open if I'm busy doing something and need to make a quick pit stop. If the girlfriend is in then I shut the door but don't lock it, she knows I'm in there and if she can hear the shower on then she also knows she's welcome to join me in there. If anyone else, family/friends/whoever are over then I lock the door automatically for anything including brushing my teeth.
I always close and lock it.

When I used to live with my room-mate then I always kept it closed, same with when I lived with my mum. Now it just depends, I don't worry about it too much if I do leave it open, but sometimes I'll find myself closing it out of habit.
I chase the cat out, who's usually in this room because the floor is warm, and then lock the door. always! Also, I live with 13 other people so it's never occured to me not to do it.
I chase my cat out too when she follows me in.
