Anyway, what should we do about Kashiwagi? Is she some form of Jester who has finally gone crazy or a Mafia? The second seems likely because of her slip up, though it could just be a ploy to get her lynched if she really is a Jester.

There might be a (town-aligned) role blocker still in play. A bulletproof townie would also make sense. I can't really think of any other reasons a mafia kill wouldn't go off. A second doctor is unlikely, I feel.

I don't think it's that unlikely. There are variations of the Doctor role after all, such as Faith Healer, which could be worked in. And this is a fairly large game. 21 people, I assume around 4 were Mafia, maybe 5 at a push, so that means 16-17 townies. Wouldn't be too much of a stretch to think there could be two Doctors in a number that large.
