FF3 final dungeon, from memory.
No save points and so if you smurf up, you have to start all the way from the beginning.
Do the Cloister of Trials count?
I never cared for the place you go to find that student ID number after visiting Deling City for the first time. But going any deeper into it is optional so I can't complain too much.
I'll start with a few notables:
FF1: Marsh Cave - this is just such a hump to get over and get into the rest of the game
FF1: Ice Cave - yay instant party wipes that I can't do anything about!
FF3: Maze+Eurika - just plain old too long. Would have been better if you could open some sort of checkpoint
FF4: Lodestone Cave - I've just gotten to the point where I run from every battle and sprint to the bottom every time. I like swords dammit.
And after that the series stopped pissing me off with specific dungeons, and decided if they were going to make something suck it would be an entire game.
I am going to go with the Ice Cave. I feel like I have to bang my head against the wall a few times to warm up for it every playthrough.

Agreed with Velo, smurf the Ice Cave. I don't really mind the Lodestone Cave though.
Also the final third or so of Final Fantasy XII. Entire thing felt like one long slog and I was just barreling my way through dungeons with a monster conga line behind me.
Anything that's just a plain cave really. Quite a few in FF6. A couple in FF10, most notably the omega ruins.
Loads of them in Crisis Core.
Anything that forces an inconvenient equipment change. Probably the magnetic cave in IV. Come to think of it, IV probably has like IV of my bottom V dungeons.
