Spoilers for all of DAII:

(SPOILER)At first, it's a full family, with Malcolm, Leandra, Bethany, Carver, and Hawke. Then Malcolm dies. Then Bethany dies while they're fleeing the Blight. Then Carver dies/becomes a Grey Warden, so it's just Leandra and Hawke. Finally, Leandra is murdered, so Hawke is the only one left.

By the end of the game, Hawke is the only surviving member of his family.
