Well, there are some that are debatable, such as:

KotR: this could be considered a reward.
Bahamut ZERO and Choco Mog: neither of these do you find lying on the ground.
Alexander: this kind of is a mini-puzzle

However, Ifrit and Bahamut you get for defeating a boss. Shiva is the only materia you HAVE to get, and it's for saving what's-her-name. You get Leviathan for defeating Godo... Odin, you get for defeating Lost Number...You get Pheonix for beating the Fort Condor battle...

So, I say 7. However, there's up to 11 possibilities, so the answer you're looking for could be any number inbetween.

Argh, I was afraid some of these would be debatable...

KotR is not a reward because you simply find it lying in the cave on Round Island. (At least, that's how I see it)

Bahamut ZERO is a reward because you need the other two Bahamuts and the Huge Materia to receive it.

Choco/Mog is a reward because you receive it from the chocobo at the ranch.

Ifrit and Bahamut are rewards from bosses, Shiva is a gift from Priscilla.

Alexander is a reward for defeating Snow, Leviathan for Godo, and Odin for the Lost Number, yes.

Phoenix is for winning the Fort Condor battle.

Eh, you mentioned them all...that's good enough.
