The way people dress is based on so many factors: societal, political, economic, ecuminical, personal, ect. The way Kuja dresses does not imply in the slightest that he is feminine. If he lived on Earth (particularly in rural Texas) then maybe he could be considered feminine, but he doesn't. He lives on Gaia in a fantasy world with different economics, politics and the such.
