I think we ban at least one member every month right now, and probably about one member a week to one of the restricted groups.
I'd say at least 100 members in all would have been permanently banned, although it's probably a lot more. I think I've permanently banned about 20-40, so yeah. Sometimes they come in batches of up to 15 or so members in one blow.
People get banned for all sorts of things, including spamming, porn-posting, flaming, trolling, ignoring staff warnings etc.
EDIT: War Angel: See the thread in the arts forum. There is a toleration of sorts for things that are obviously artistic, such as a Roman statue, but I guess it's all about the context in which you post it. We've yet to come across anything that wasn't obviously out of grounds, or at least if we have I've forgotten about it, and if we do I'm sure a thread will be made about it and we'll decide based on what we think. If you're ever unsure about whether or not you should post something, you should ask an admin or CK and they'll tell you if it's okay or not.
