It's definitely Thief Hat; it gives a +2 to speed. Green Berets and Flash Hats are only +1. P.S.: Your other items were right.
And as for the PA boost items: (Weapon) Mace of Zeus, (Helmet) Twist Headband, (Armor) Power Sleeve or Robe of Lords, (Accessory) Bracer.
And I believe Monks get the best PA bonus of any class.
I, however, will wait for confirmation and for someone to answer the first question above.
