What do you mean by activating? There's nothing to activate.
Equip it, and the sword will become stronger as your character's HP goes up. The higher the current HP, the more powerful it is.
Oh, but it says it will transform. Whats up with that?
Means it gets stronger as you get stronger. Nothing more than that.
The sword looks different as it grows stronger. It lengthens and gets a blue tint to it. Nothing spectacular.

Yup, you need to have a lot of meat to wield it effectively. I suggest you visit your dietician and the gym trainer on the details.
