Never. I consider 'near-death'. Literally near death. Like I should have died but didn't.
The closest was driving straight into an oncoming car from the right, only to swerve at the last moment reflexively.

It was a while ago; I was on a small boat and it was very very stormy... Huge waves. It was hella scary. We turned around after deciding we wouldnt be able to get through it.
