"The world was expecting us to bring her back to life as this is the classic convention. But we did not. We had decided this from the beginning. There was a lot of reaction from Japanese users. Thats funny, because I read another article where it said Aeris wasn't originally intended to die, and that thats who Cloud would have ended up with, and they confused the staff when they suddenly decided to kill her. So I think its odd that they would say that they decided it from the beginning. ALso, there was another cave that could only be found in the Debug Room of the PC version of the game, where appearently Aeris was originally intened to die. There is a possibility that that cave could be the entrance to that aswell.
