Yes, "General Ivan" is just a nasty remark referring to the fact that Ocelot is Russian. Vamp calls him "Crazy Ivan" in MGS2, as well, but it's not his actual name. It's the same as calling an American guy "Johnny," a Mexican guy "Miguel," etc.
