Assuming you have the necessary Limit Break manual, and have learned the character's previous Breaks, using the item on the correct character should teach you the final Limit. As Mercen suggested, this requires having learned six previous Limits for the listed characters. For Vincent (who has only one Limit per Limit Level), Chaos can be learned as soon as he has learned the previous three. Cait Sith has no Limit Breaks beyond Level 2, and therefore no Level 4 Limit ability or item.
Unfortunately, it is possible to be incapable of learning the final Limit, even if you have met the intended requirements. There are a number of Limit Break-related glitches in FFVII, most of which are wholly detrimental. In your case, you may have been the victim of a glitch whereby it is simply impossible to learn Level 4 Limits. As far as I know, your only options are to start a new game, or to continue on without learning or being able to learn the Limits.
