2-Boy at Balamb near the beggining of the game while looking at world map with a girl.
3-Two.At the excavation dream by Laguna and by the old man on Dollet to his grandson (because he put a bone on the painting).
4-See the armor at Winhill and Tomb of Unknown King.The Unknown king dresses it.
5-Martine says it at the entrance of the Galb.Garden and Laguna at Deling city Hotel.
6-Desert Prision and Lunatic Pandora Laboratory.
7-Missile base and Lunar Gate.
8-On Timber after the resistance train and on the Desert Prision Station where our characters scape to Balamb.

(If I my guess about how "Sam Hill" is used in the game is right I am going to be laughing at Future Esthar for a week straight...)
