Judging from those scenes that they found without any other stuffs to support it, it is still hard to judge that she is originally planned to die at the northern crater.
And bear in mind kiddies, she did died in the forgotten capital and that is a fact. If you want, you can go investigate those stuffs they have there for so long without any updates.

The ghost thing is probably a glitch, since it said that it appears when she is kidnapped/dead must mean that whenever she is not in your PHS party she appears.
Under the assumption that there is no reason to return to the church after Aeris joins you probably means that the devoloper's didn't check the church after she dies and didn't fix it because they never visited the church afterwards.

One word: Patches.
PC game developers are notorious for flaws in programming that they have to release patches to fix bugs and glitches in their games.
It's reasonable to expect the FFVII programmers to have made an error in their quality control, like most other developers seem to do.
