As well as the standard stuff; character development, a unique storyline, impressive and creative gameplay style, you have to remember the little things. A game with a sense of humour is always great fun! and I can't speak for anyone else here, but thousands of pointless quests and side-quests as well as bosses that are completely impossible tend to get to me.
