Does it only start to work when your near death in battle?
In a way, yes, Angel Heart, but there's more to it. If no-one gets it by tomorrow I'll reveal the answer and someone else can have a go.

In battle, if a character gets 'Near Death' status (HP number value is in
yellow rather than white numbers), their HP are restored.

No-one got it, so anyone can ask a question. Restore HP will recover 50% of the character's maximum HP if they are attacked while they have critical HP.

You sure? The first is Aries, then Cancer, Gemini, Taurus and then Libra. The reward for the fifth Stellazzio is 5,000 gil.
Positive, Libra's not the fifth one you come across.
Ah, it's the seventh (add Scorpio and Virgo to the list), so that's 10,000 gil.
