how do you do near death attacks

I've only ever seen a couple of them, but that's partly because my characters are very rarely near death. I did get Locke's in the colliseum once, which was nice.

I've played through the game at least a dozen times, and I've only seen Sabins. I saw it in the colliseum.

I've gotten Terra's and somebody else's before when I was trying for it. I think one time I was fighting regular enemies, but this was a long time ago, so I can't remember too well. I think I remember using Terra's on Kefka once, but when I tried to do it again he killed me. I've seen Terra's a few times.
