Chaos: Recruit Vincent and visit the waterfall cave with a mountain chocobo or once you get the submarine. You should see a scene with Vincent. Return later in the game, again with Vincent, and you should see a different scene. At that point you should get both Chaos and Death Penalty.
Highwind: Use the submarine to find the crashed Gelnika (airplane). It's in a chest in the cargo bay.
Final Heaven: With Tifa in the party, go to her house in Neibelhiem and play the song he remembered. X, [], Triangle, R1/L1+Triangle, R1/L1+[], X, [], Triangle, R1/L1+X, O, X, [], X. You've done it right if it sounds like the Highwind theme. Tifa should then mention something about sheet music and finds her limit.
Venus Gospel: Talk to the old man in front of the item shop several times after the rocket launch in Rocket Town. He'll eventually hand it over.
Premium Heart: Dig up the Key to Sector 5 in the bone village. Return to Midgar and Wall Market. Go to the broken automated dispensing machine. It will work long enough to dispense it.
Death Penalty: See directions for Chaos.

Vincent must be in your party of 3, and you must visit in disc 2. most people wait until disc 3 before going back in to claim your prizes, but it is possible to go back in simply after fighting about 20 battles on land.
