If you go to the basment in the Nibelheim Mansion, you will get a scene. There is another scene on the "Reminisence of FF7" feature on the Advent Childen DVD where it shows Zack being shot in the face with a rifle.
Note that you can't see the flashback scene in the basement of the Shinra Mansion in Nibelheim until you've (SPOILER)got Cloud back from the Lifestream. It's a pretty good cutscene that adds some depth to Cloud and Zack's character relations.

It pretty much spells out for you through out the game that Zack is VERY dead, then again you see Rufus get blown to bits and Tseng stabbed violently by Sephiroth and they both came back, so who knows
