I figured that it was totally isolated and that they couldn't leave if they tried. They didn't wonder about being the only town because they don't have any other towns to compare to. Like when you were born, you didn't understand cities or anything, the only thing that exists is what you see. It's like that for them, the only town that exists is the one they live in.
You guys are confusing Dream Zanarkand with the real Zanarkand. The real Zanarkand obviously knew of other cities, and those cities were in the prime of innovation with machina as well. Though Dream Zanarkand is just a dream, no where else was included.
