I don't know how some of you are getting these rediculous chains. I have all 3 for everyone, and the most I've gotten is 12 or 13.

I think I've only gotten like 12 or so. I'm not known for my reflexes, what can I say.
Well, luck determines your success many times. Sometimes I've gone into the quickenings with all 3 characters at 3 mist charges, and after the first person's quickening I couldn't get either of the other 2 characters' quickenings to show up despite spamming R2. Other times, and actually my best chain(19) occured this way, you can go in with only one mist charge on one person and get lucky and get a huge chain. I think certain boss fights are especially prone to making chains harder/easier to get. My chain 19 came out of desparation because I was about to die and spammed the quickening to get in one last shot. After Vaan's initial Red Spiral, Ashe kicked off the craziness with 3 Maelstrom's Bolts in a row, after which 1-2 of Vaan, Baltheir, or Ashe's quickenings were available immediately after the previous quickening. Only once did I have to even R2 to get a Mist Charge to show up, to get 12-13. I think there were 6 lvl 3's 5 lvl 2's and 8 lvl 1's, so they busted out Black Hole and saved my butt. So there's definately an element of luck involved. Other than that it's all about reflexes. Don't waste time early on because the faster you get your commands in, the more time you'll have for the next quickening.
31 chained together (I usually get 20, which I must admit, is really bad because my fingernails are about to fall off). It was hilarious, yet utterly useless that time, because (SPOILER)Cid only had 19000 HP left, I did 99999 damage, and I didn't even need help (SPOILER)from Reddas.
I was not fortunate enough to get a Black Hole, but I did get a Thunder-based attack, called Ark Blast. Or something similar. I've only seen Torrent, Cataclysm, and Inferno (despite finishing the game).
I've gotten 20, but it usually stops at 14.

i think the amount of times you can do it isnt jsut based off of luck. but based off of how often you use quickenings. after i went a long time without useing a quickening i got 15 then i tried again 5 minutes later and got 3 so it might be based on the amount of enemys you kill or something else or the fact that i got 15 then 3 might just have been bad luck i dont know. but its possible i guess.

it seems to me that i have a # based conccurence.ill explain,5-8MCis inferno.9-11MC ark blast somtimes cataclysm.12-14MC usually torrent.15-19MC lumineccense.19 is as far as ive gotten now.it doesnt always end up like that i get the wind one occasionaly but not enough times to tell how many MC's it takes.

i kinda noticed that too. For me level 1 mist charges: 1-6 is inferno, 7-10 was catacylism, 10-13 was wind blast, An for level 2 mist charges( i havent got level 3 yet) 1-7 was inferno, 7-9 was catacylism, 8-12 was tourrent, an sumtimes 12-17( 17 is the highest i got so far) was the ark blast.

Also even if you get seven lvl. 1, five lvl. 2, and two lvl. 3 that could count for five different concurrences, but will always result in Ark Blast.
Another thing which almost all of you probobly know but I just want to point out that none of them cause elemental damage even if it sounds like it does, they just do physical damage.
Resistances don't effect quickenings also, the only thing that can resist a quickening is a creature with an immunity to physical damage.
