There's two different kinds of fashion, avant garde and ready-to-wear. Ready-to-wear is obviously just as it sounds, clothes meant to be sold in mass production and worn by people everywhere. Avant garde fashion CAN'T be mass produced. They're individually crafted works of art. What happens when a designer creates these amazing designs is that later, they're translated to more mainstream wear.
Project Runway did a couple challenges like this, create an avant garde look, and translate it to ready-to-wear.

Alexander McQueen was so popular because he really was an incredible artist. He pushed boundaries, did things visually with clothes that other people weren't doing.
