In my experience, being hit in the balls doesn't hurt as such, but it instinctively makes them retract and puts various defense mechanisms through your mind that are similar to pain without the actual pain.

I guess for women it would hurt more if you were to hit them anywhere around the pelvic area when it's their time of the month. They experience all sorts. Groin pains, backache, similar stomach aches except it's not the actual stomach it's more the womb.
